We partner with our Nurses at St. David's Medical Center!Job Summary and Qualifications The Discharge Nurse (RN) utilizes the nursing process to direct and deliver comprehensive nursing discharge instructions to patients in discharge suite in a safe and proficient manner according to individual patient needs. This position is responsible for preparing discharge documentation, educating patient on discharge instructions, mitigating any discharge barriers for patients discharging home, and answering any discharge questions from patient or family members. This position will assist in the coordination of post-hospital needs of all patients going home in order to promote quality, continuity of care and safety for the patient. The Discharge RN will meet the needs of the patient to help facilitate a timely and efficient discharge from the hospital. The Discharge RN will assist in patient advocacy and provide any direct patient care needed based upon St Davids Medical Center Standards of Care and Standards of Practice, Polices, Procedures and Practice Guidelines. Facilitates the discharge of patients from inpatient, emergency room, or outpatient surgery to the discharge suite by coordinating with facility RNs, and Charge Nurses. Patient care is delivered in accordance with assessed needs.Prepares, explains, and validates understanding of discharge instructions by patient and patient's family members if present.Documentation is consistent, complete, thorough and legible. Documentation includes discharge instructions, education resources, department audits and department logs.Patient care is administered based on Hospital and Departmental specific standards of care and practice.The RN involves the patient and/or significant other(s) in the patient's care. Education/instruction are provided specific to the patient's health care needs and age requirements.Physician orders for all assigned patients are reviewed & carried out accordingly.Accepts only physician's verbal or telephone orders in accordance with practice guidelines using 2 patient identifiers. Requirements Graduate of an accredited school of nursing with a BSN degree. License and Certifications Registered Nurse license by the Texas Board of Nurse Examiners. If a compact license is held, RN is required to obtain a Texas RN license within 90 days of their hire date.BLS is required per St. David's HealthCare Policy.5 years of nursing experience in an acute care setting is required.Previous computer experience required.This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved). The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
